Just execute 'SETSOUND.EXE' in the 'SOUND' directory and configure the sound itself. Make sure to set 'Creative Labs Sound Blaster Pro (New Version)' on 'MIDI Music' and 'Creative Labs Sound Blaster 16 or AWE32' on 'Digital audio'.
Theme Hospital works fine on Version 0.74. If you copy the cds content to your pc, start the game from dosbox or a launcher, running the HOSPITAL file. The final option which I eventually resorted to using is to install Theme Hospital to DOSbox. This works on all Windows versions from 2000 up.
If u have done this start the game normally and the sound will work nice and smooth. I used the orignial Theme Hospital with the one offical patch (from Bullfrog itself, not that community one).

You could try going to the SOUND folder in the games directory and click on SETSOUND to set your midi and digital audio options in windows. After that the game should be ok (it was the solution for my launcher). The slowdown seems to have been largely caused on this machine by the display scaler being on, so turn it off if your having issues. In dosbox game launcher I have the display set to 'surface' and the sound to 'sb16'. (2011-08-12 10:47) vaniilabear how do you download it then?
Still having speed issues (2010-06-09 04:46) crho85 Although it appears there should be no problems with the latest version. I am still having very slow playback, even with frames turned up 75000+. A 2.61 GHz duel core should be able to run this. I set core to dynamic as well.

But its annoying to play the game because its running slow (on my 3ghz.). Sound worked with no problems after running that sound setup BAT file. For the books (2003-11-01 18:12) millionmice Sound support for theme hospital has always been iffy, even under DOS. Speed was good. Sound support seems iffy.
